For hospitals navigating the intricacies of capitated contracts—where fixed payments are made irrespective of services rendered—the role of a doula presents a compelling financial argument. Evidence suggests that doula-assisted births may lead to fewer medical interventions, shorter labor durations, and reduced postpartum complications. These reductions not only enhance the childbirth experience for mothers but also translate into tangible cost savings for hospitals. Doulas: Doulas are non-medical professionals trained to provide emotional, physical, and educational support to mothers before, during, and shortly after childbirth. They complement the medical care provided by doctors and nurses.

Capitated Contracts: In the healthcare setting, a capitated contract is a payment agreement where providers (like hospitals or doctors) receive a set amount (a “capitation”) for each enrolled person assigned to them, per period of time, whether or not that person seeks care. The amount is pre-determined and is meant to cover all the standard services the patient might need during that time frame.

Financial Benefits of Doulas for Hospitals with Capitated Contracts:

  1. Reduced Interventions: Studies have shown that the presence of a doula during childbirth can reduce the need for certain medical interventions, such as cesarean sections, forceps, and vacuum deliveries. For hospitals under capitated contracts, the financial incentive is to provide care efficiently while maintaining quality. It’s worth noting that while the financial benefits are a positive outcome, the primary goal of integrating doulas into maternity care is to enhance the childbirth experience and outcomes for mothers and babies. Profitability

This category evaluates the ability of a hospital to generate a surplus.

  • Operating Margin (ratio of operating income to total revenue)
    Definition: Operating Income/Total Revenue
    Operating income is income from normal operations of a hospital, including patient care and other activities, such as research, gift shops, parking and cafeteria, minus the expenses associated with such activities. Operating Margin is a critical ratio that measures how profitable the hospital is when looking at the performance of its primary activities. A negative Operating Margin is usually an early sign of financial difficulty.
  • Non-Operating margin (ratio of non-operating income to total revenue)
    Definition: Non-Operating Income/Total Revenue
    Non-operating income includes items not related to operations, such as investment income, contributions, gains from the sale of assets and other unrelated business activities.
  • Total Margin (ratio of total income to total revenue)
    Definition: Total Income/Total Revenue
    This ratio evaluates the overall profitability of the hospital using both operating surplus (loss) and non-operating surplus (loss).

Liquidity

This category evaluates the ability of the hospital to generate cash for normal business operations. A worsening liquidity position is usually a primary indication that a hospital is experiencing financial distress.

  • Current Ratio (ratio of current assets to current liabilities)
    Definition: Total Current Assets/Total Current Liabilities
    This ratio measures the hospital’s ability to meet its current liabilities with its current assets (assets expected to be realized in cash during the fiscal year). A ratio of 1.0 or higher indicates that all current liabilities could be adequately covered by the hospital’s existing current assets.
  • Average Days in Accounts Receivable (ratio of net patient accounts receivable to total revenue/365)
    Definition: Net Patient Accounts Receivable/(Net Patient Service Revenue/365)
    This ratio measures the average number of days in the collection period. A larger number of days represent cash that is unavailable for use in operations.
  • Average Payment Period (ratio of current liabilities less estimated 3rd party settlements to total expenses less depreciation and amortization/365)
    Definition: (Total Current Liabilities-Estimated 3rd Party Settlements)/ [(Total Expenses-(Depreciation Expense + Amortization Expense))/365)]
    This ratio measures the average number of days it takes a hospital to pay its bills.

Solvency/Capital Structure

This category evaluates the health of a hospital’s capital structure, measuring how a hospital’s assets are financed and how able the hospital is to take on more debt. Both measures are critical to the hospital’s long-term solvency.

  • Debt Service Coverage Ratio-Total (ratio of total income plus interest expense plus depreciation and amortization to interest expense and current portion of long term debt)
    Definition: (Total Income + Interest Expense + Depreciation Expense + Amortization Expense)/(Interest Expense + Current Portion of Long-Term Debt)
    This ratio measures the ability of a hospital to cover current debt obligation with funds derived from both operating and non-operating activity. Higher ratios indicate a hospital is better able to meet its financing commitments. A ratio of 1.0 indicates that average income would just cover current interest and principal payments on long-term debt.
  • Cash Flow to Total Debt (ratio of total income plus depreciation and amortization to total current liabilities plus total long-term debt)
    Definition: (Total Income + Depreciation Expense + Amortization Expense)/(Current Liabilities + Long-Term Debt)
    This ratio reflects the amount of cash flow being applied to total outstanding debt (all current liabilities in addition to long-term debt) and reflects how much cash can be applied to debt repayment. The lower this ratio, the more likely a hospital will be unable to meet debt payments of interest and principal and the higher the likelihood of violating any debt covenants.
  • Equity Financing (ratio of net assets to total assets)
    Definition: Total Net Assets/Total Assets
    This ratio reflects the ability of a hospital to take on more debt and is measured by the proportion of total assets financed by equity. Low values indicate a hospital has used substantial debt financing to fund asset acquisition and, therefore, may have difficulty taking on more debt to finance further asset acquisition.

Other Measures

The following items are individual line items from the Quarterly Financial Statements.

  • Total Surplus (Loss): Total dollar amount of surplus or loss derived from all operating and non-operating activities.
  • Total Net Assets: The difference between the Assets and Liabilities of a hospital. Comprised of retained earnings from operations and contributions from donors. Changes from year to year are attributable to two major categories (1) increases (decreases) in Unrestricted Net Assets (affected by operations) and (2) changes in Restricted Net Assets (restricted contributions).
  • Assets Whose Use is Limited: The current and non-current monies set aside for specific purposes, such as debt repayment, funded depreciation and other board designated purposes. Board-designated funds are most readily available to the organization as the board has the ability to make these funds available if needed. This is a valuable measure because it reveals potential resources that the hospital may have available for cash flow if necessary.
  • Net Patient Service Revenue (NPSR): Revenue a hospital would expect to collect for services provided less contractual allowances. Net Patient Service Revenue is the primary source of revenue for a hospital.

Opioid Crisis Response: Key Opportunities and Challenges for Hospitals and Health Systems

On October 24, 2018, President Trump signed H.R. 6 into law. More formally known as the Substance Use Disorder Prevention that Promotes Opioid Recovery and Treatment (SUPPORT) for Patients and Communities Act, H.R. 6 is a follow-up to the last bipartisan opioid crisis-focused legislation, the 2016 Comprehensive Addiction and Recovery Act (CARA).

Like CARA, the SUPPORT Act takes sweeping aim at the opioid crisis, focusing on numerous aspects of opioid prevention, treatment, and recovery, including expansion of opioid use disorder (OUD) treatment access and capacity in residential and inpatient care, medication assisted treatment (MAT), and via telehealth and improving medical education and training resources for health care providers to better address addiction, pain, and the opioid crisis.

The takeaways for hospitals and health systems from H.R. 6 include both important specific requirements and strategic trends in addressing opioid related activities.

Hospitals and health systems should pay particular attention to new regulatory and reimbursement requirements, as well as the evolution in best practices reflected in the SUPPORT Act.

H.R. 6 highlights an investment into and focus on other critical social determinants relevant to OUDs.

  • Section 7183, the CAREER Act, is intended to improve resources and wraparound support services for individuals in recovery from a SUD in the transition from treatment programs to independent living and reintegration into the workforce.
  • Section 7031, the Ensuring Access to Quality Sober Living Act, requires HHS to develop best practices for operating recovery housing (shared living environments free from alcohol and illegal drug use and centered on peer support and connection to services that promote recovery from substance-use disorders).

The ability to focus on social determinants appears to be an essential piece of supporting the initial decision to seek treatment and preventing relapse. At the same time, traditional reimbursement mechanisms do not provide funds to meet these needs.

Hospitals and health systems should consider opportunities to identify resources offering housing and transitional support for patients in treatment and recovery from OUDs and to provide information and as seamless a transition as possible.

Perhaps the biggest opportunity for all health care organizations is to improve training to prevent opioid dependency.

  • Section 7101 of H.R. 6 expands medical education and training resources for health care providers to better address addiction, pain, and the opioid crisis.
  • Section 6092, the Combating Opioid Abuse for Care in Hospitals (COACH) Act, requires CMS to publish guidance for hospitals on pain management and OUD prevention strategies for Medicare beneficiaries.

Additional lessons for hospitals are likely to be forthcoming.

  • Section 6104 prohibits hospital patient pain surveys (unless the questions address the risks of opioid use and the availability of non-opioid alternatives).
  • In their place, the Treatment, Education, and Community Help (TEACH) to Combat Addiction Act, Section 7101, requires SAMHSA to designate Regional Centers of Excellence in SUD Education to improve pain management and SUD education by developing evidence-based curricula for health care professional schools.
  • Section 7121 also requires SAMHSA to award grants to establish or operate at least ten comprehensive opioid recovery centers across the country to conduct outreach and provide a full continuum of treatment and recovery services, including job-placement assistance.

Training also extends to Medicare beneficiaries:

  • Section 6021 requires CMS to provide Medicare beneficiaries with educational resources regarding opioid use and pain management, as well as descriptions of covered alternative (non-opioid) pain management treatments.

Some hospitals have focused specific training efforts on naloxone administration, enabling greater numbers of patient family members and loved ones, as well as first responders, to act quickly in response to overdoses.

H.R. 6 is extensive. It highlights legal changes not only related to evolving compliance requirements but also to best practices in reducing opioid risks and improving outcomes in treatment of opioid and other substance use disorders.
